This is a simple form which can be used by executors and legal personal representatives to notify creditors of the death of the deceased. As well as providing notice of the death, the form also includes a request to close any credit accounts held by the deceased person. The form can be used to inform book stores, merchandisers, credit card companies, retail stores, credit and lending institutions, mortgage companies and other businesses with which the deceased had an account of the death of the deceased person.
This request is very important because executors have a duty to take reasonable care to prevent the depletion of the assets of the estate pending completion of the probate process. It is important therefore, in terms of preventing the depletion of the assets of the estate, to close off any subscription accounts or other automated payment accounts which the deceased may have had.
This should also ensure that any correspondence from the creditor will be properly addressed to the executor. This, in turn, should ensure that a situation does not arise where the deceased’s estate continues paying for poducts or services that it does not require.
