A Notice of Revocation of a Power of Attorney is used to terminate an existing power of attorney for property and finance or an existing power of attorney for healthcare. In the absence of properly revoking an existing power of attorney, your agent may be able to make financial and/or medical decisions on your behalf and to bind you to those decisions.

This Notice of Revocation of a Power of Attorney can be used by someone who has previously made a power of attorney.
