Advance Health Care Directive Packages which include a Power of Attorney for Health Care and a Living Will Form for use in all states. A Health Care Power of Attorney is a legal document that allows you to designate a person (an "Agent") who will have the authority to make health care decisions on your behalf if you are unconscious, mentally incompetent, or otherwise unable to make such decisions.

 

Durable General Power of Attorney

The essential difference between a durable power of attorney and a standard power of attorney is that the authorization under this power of attorney will continue in force during your incapacity. With standard powers of attorney, this is not the case and the authority is deemed to be revoked automatically.

The essential difference between this Limited POA form and the above Durable General Power of Attorney form is that the scope of authority granted to your agent can be limited in terms of the activity to be carried out, to a specific transaction, limited in time or in any way you wish..Like the above form, the authority is not deemed to be revoked upon you becoming incapcitated or ill.
